SAHK New Territories West Integrated Community Rehabilitation Centre
Objectives
The Centre aims to provide a continuum of coherent and interconnected services for persons with disabilities having intensive care needs and living at home and to provide support to their carers. We will provide integrated day care and home-based support services to be delivered in a flexible manner which will cater for the actual impairment level and changing needs of the persons with severe disabilities.
Features
- Providing integrated centre-based and home-based rehabilitation services;
- Using advanced technology products and assistive technology for rehabilitation;
- Applying the International Classification of Functioning, Disability and Health (ICF) framework in devising a structure comprising rehabilitation objectives, intervention, assessment, and evaluation to facilitate data and experience sharing among rehabilitation organisations; and
- Co-operating with Jockey Club Rehabilitation Seating Service Centre# in order to provide one-stop rental service for rehabilitation products.

Target Service Users
The target service users (with a limited number of persons with moderate disabilities) are the persons with severe disabilities aged at 15 or above living in Tsuen Wan / Kwai Chung and Tsing Yi / Yuen Long and Tuen Mun
- suffering from severe disabilities for intellectual and/ or physical (including stroke, neurological or physical impairment); or
- referred by medical officers/occupational therapist /physiotherapist or assessed by the Centre as suitable for the Services; and
- Carers of the above-mentioned service users
Application Procedure
The service users can be referred by social workers / medical personnel or apply for the service with the Centre directly.
Withdrawal from Service
Service users can withdraw from the service anytime for personal reasons. When the service is no longer suitable for the users, the Centre may propose termination of service and make appropriate referrals for alternative support for the users.

〜For service users with financial difficulties, please contact the Social Worker to apply for the half fee-reduction.
Service Hours
Monday to Saturday: 8:30am to 5:30pm
(Closed on Sundays and Public Holidays)
