Continuing Rehabilitation Service

Since 1997, the Association strived to design a rehabilitation system which is suitable for taking care of chronic ill patients. With the funding support from the S.K. Yee Medical Foundation, we developed the "Holistic Approach to Rehabilitation” which is compatible with the local culture and resources. This approach is not a treatment method, but rather is a rehabilitation system based on "person-centred”. Through the transdisciplinary team approach, a structured environment and integrated training based on daily living, the different aspects involving "physical, intellectual, and psycho-social” of the clients will all be focused on so as to arouse the client’s motivation to self help and social integration.

  • Continuing Rehabilitation Centre

On 6th January 2006, the Association’s first self-financed Continuing Rehabilitation Centre started operation at a premises in Tai Hang Tung Estatein Shek Kip Mei. It aims to serve community dwelling, neurological impaired chronic patients such as stroke, Parkinsonim, dementia etc. by providing them with a tailor-made rehabilitation programme.

The principal of the Centre is to emphasize the importance of the home and the community in assisting the family members to understand the conditions of the clients and to learn the related training and management skills. The client will learn to self-help, as far as possible, reduce reliance and maintain their conditions. The Centre also organized various seminars, talks and workshops to promote the "Holistic Approach to Rehabilitation” to other organizations, and provide a transition to other community support programmes. The approach is also applied in day centres for the elderly, care and attention home and also rehabilitation hospitals.

Scope of Service

Mode of Service

The Centre uses group training approach, however if after assessment, individual training will also be provided. There is also various equipment and aids, so as to offer a diversified training. There will also the provision of orthotic and prosthetic service and special transportation service.

Home-based Visit

Therapist can visit the client in their home to provide assessment and training so that the training learned can be applied into the living environment. The experienced therapist will also provide professional advice to make home alterations so as facilitate the family members in taking care of the client.

Outreach Service

Visit will be made to care and attention home for the elderly to provide professional therapeutic service, including assessment, individual or group treatment and also to provide in-service training to the staff there. Professional advice will also be given to design and modifications to the environemtn.

 

  • New Territories East Community Rehabilitation Day Centre

The Centre caters for discharged patients from the hospital who suffers from stroke, nuerological impairment, physical disability or other impairment to mobility as a result of sickness. A comprehensive and continuing rehabilitation service will be provided to improve and maintain their mobility and self care abilities, thus minimizing their dependency and deterioration process. Through therapeutic treatment and support, the clients will be assisted to rebuild their daily living schedule and mode. This will better prepare them to integrate into the society and at the same time training will also be provided to the carers to help them to carry out the care and training at home.

The Centre also provides day training centre for severely disabled persons, aged 15 to 59, including basic care and nursing services, social and recreational activities and maintain close network with the community. The measures will assist the clients to continue living in the community and relieve the stress of the family members and the carers.

Target Clientele:

  • Recently discharged patients from the hospital : referred by the medical doctors, physiotherapists, occupational therapists from Hospital Authority;
  • Training for Carers/Respite day service for severely disabled persons : referred by social workers, medical personnel or self application.
