Aim of Service

The Jockey Club End-of-Life Community Care Project: Unison is an initiative funded by the Hong Kong Jockey Club Charities Trust. Its goal is to provide physical, mental, social, and spiritual support for persons with disabilities who are approaching the end of life, as well as for their caregivers. The project aims to help patients spend their final days with dignity.

Project Objectives

  • Improve the quality of life for patients in the late stages of illness.
  • Reduce the psychological burden on caregivers.
  • Enhance staff knowledge and skills in providing end-of-life care.
  • Raise public awareness about end-of-life services available for persons with disabilities.

Service Scope and Features

  • Case Management:A team consisting of Social Worker, Nurse, and Therapist serves as case managers to create personalised care plans for persons with disabilities who are facing terminal illnesses. They provide palliative and end-of-life care services.
  • Caregiver Support:Organise group activities that offer psychological counselling and emotional support for caregivers. This includes grief counselling and assistance with end-of-life arrangements as needed.
  • Staff Training:Provide training for frontline and professional staff to enhance their understanding and skills in delivering end-of-life care for persons with disabilities.
  • Community Education:Train volunteers interested in supporting terminal patients and enhance public awareness of end-of-life services through public seminars.

Target Group

  • Persons with terminal illnesses or deteriorating neurological diseases and their caregivers are eligible for assistance.
Applicants who are currently using the Association's Adult Services ( residential service ) or community support services are also eligible. Additionally, support from non-governmental organisations, hospitals, or self-help groups located in New Territories East can be considered.

40 persons with disabilities and their caregivers annually

Referral by Social Worker.

Service Hours

  • Monday, Tuesday, Thursday & Friday: 9:00 am to 6:00 pm
  • Wednesday: 12:00 pm to 8:00 pm
(Closed on Saturdays, Sundays, and Public Holidays)
