Community Support Service

The quality of life of disabled persons is an issue that draws more and more attention. The Association puts much effort in recent years to develop community rehabilitation services to the disabled in the community and through supporting their families, the disabled can enhance their living skills, integrate into the society and start a new page. At the same time, we encourage the carers of the disabled to actively participate in the training, and learn the appropriate skills so that the disabled can continue their rehabilitation at home.

We also provide tetraplegic clients with transitional residential service as well as day-time training, so as to assist our clients in adjusting and re-integrate into the community

The Association pioneered in the setting up of Home-based Rehabilitation service in 1980 and in 2006 and 2008, in the setting up of Continuous Rehabilitation Centre and Community Rehabilitation Day Centres. In addition we also set up the first centre in the whole of South East Asia to provide transitional residential service as well as day-time training for tetraplegic clients. In 2023, the Association had been awarded by SWD and set up New Territories West Integrated Community Rehabilitation Centre. The Centre aims to provide a continuum of coherent and interconnected services for persons with disabilities having intensive care needs and living at home and to provide support to their carers. We will provide integrated day care and home-based support services to be delivered in a flexible manner which will cater for the actual impairment level and changing needs of the persons with severe disabilities. Moreover, Jockey Club End-of-Life Community Care Project: Unison was funded by the Hong Kong Jockey Club Charities Trust in 2024. Its goal is to provide physical, mental, social, and spiritual support for persons with disabilities who are approaching the end of life, as well as for their caregivers. The project aims to help patients spend their final days with dignity.
